Organizations can fill out the referral form found on this page and faxed to our number 833-975-0724. A recent H/P would also be appreciated. Note there are separate forms for RPM/CCM and Primary care.

Individual patients can self refer by calling the main office at 317-349-7680 and speak directly with our office staff.
